This is a little off-topic but we've thought of having an
"evaluation exam" also. What goes into your "evaluation exam"?
Does it change from year to year?

We made the exam up with the idea of trying to find how much of the
"big" ideas a student has retained, rather than details. We assume the
student hasn't necessarily thought about the physics in about a year.
Thus, the questions may look easy for a student who just finsihed the
course. For example, can they correctly calculate an acceleration? Do
they know about conservation of energy/momentum? etc.

No, it doesn't change from year to year.
