Our "Student Evaluation of Laboratory Experiences" has eleven "on a
scale of 1 to 5" statements. 5 is superior, 3 is average, 1 is
unsatisfactory. The evaluation is used for all laboratory (or clinical)
classes on campus; not just physics. The statements are:

1. Makes assignments clear prior to beginning the experience.

2. Assignments relate to current course concepts.

3. An orientation to each assigned experience is provided.

4. Expectations of student performance are made clear.

5. Procedures and/or techniques are correctly demonstrated or
explained.

6. Progress is carefully supervised.

7. Successful participation of students is encouraged.

8. Student self-confidence is developed.

9. Feedback is provided after completion of each laboratory experience.

10. Evaluation techniques are fair and accurate.

11. Treats the student courteously.

On the back of the form, students are asked to discuss strengths and
weaknesses relative to the following topics:

1. Prepares clear assignments.

2. Variety of learning experiences.

3. Helpfulness of related clinical agency staff and/or technicians.

4. Equipment is up-to-date and in good working order.

5. Necessary equipment is available.

6. Student safety provided for.
