Let me speculate:

This note looks like it came from me - Brian: but it did not.
It came from a list server, which inserted my text into
a form header and stripped out attachments and graphics.

The server in question serves many lists and may well
consist of heterogeneous systems.
That is the first possibility - variegated server systems.

Then this list is nominally moderated by Dan - and it may be,
by his deputies. This review method may insert a domestic,
Microsoft element into what could otherwise be unix/linux
software & hardware elements - and judging by the
general robustness, probably are.
That is the second possibility - list moderator reviews.

I expect there are two or three other channels for text
access and line formatting too...

I wish I could. It happens on this list more than on others I am
subscribed to, but even here it is pretty much random. Sometimes it
shows up, and sometimes is doesn't. I have not been able to see any
pattern to it.
