Another possibility is to use LoggerPro itself. It is not very
sophisticated, but you can add pages to a LoggerPro document where you can
insert text and pictures.
These could be prepared ahead of time with the questions and some textboxes
for answers. Then the .cmbl file can be uploaded to whatever CMS is used (or
attached as email)
