Q-PSI: Quantum Processes, Systems, and Information by Benjamin
Schumacher and Michael Westmoreland, Cambridge University Press, 2010.

There are others, but this one is pretty good. It's the one I'm using
and, until I write a better one, is the best out there. I have also
successfully adapted Modern Quantum Mechanics by JJ Sakurai (Addison
Wesley, I think) for undergraduates, but it can be a lot of work.

Bohm wrote Quantum Theory (available via Dover), but it takes the
Copenhagen line since it came out prior to his "conversion" to
non-Copenhagen-like thinking.
