How did reporting "human
error" as a source of error become so prevalent? Do any of you want to
defend it as a reasonable thing to write on a lab report (or in a
publication)?

It must be in middle school because they do it in HS and none of my
colleagues will "fess up" to teaching or tolerating it. :-)

I try, mostly in vain, to have my students detail what specific part of
their inherent erroneousness are they talking about? "So what specifically
did you do wrong this time?"

I feel you pain and frustration.
