The CPT theorem assumes that a local field theory is the ultimate
theory. If the fundamental theory were nonlocal (and perhaps bizarre in
other ways as well), it would be possible to violate CPT conservation.

That being said, there is good empirical evidence the CPT is a good
symmetry, and very few expect it to be violated.
