I use one for informal feedback that is a simple, half page which has:

Circle the appropriate response (each of three questions has a 1 to 10
scale after it)

How well did today's lesson improve your physics knowledge?

How well did today's lesson improve your problem solving skills?

How well did today's lesson motivate you to use what you have learned?

(you can replace "today" with "this week's" or other appropriate time
span).

Following this are three open-ended comment sections:

What you liked:

What you didn't like:

What you would change:



The form works well for quick feedback from students in class ... opens
door for getting more feedback and modifying courses on the fly.

We also have a nice formal instrument given at the end of terms ... can
FAX to you if interested.
